Does anyone know of an AI story where a mistake in the input severely affected the expected output?

Long story short I have to write a speech and I want to use the overarching theme of “life is what you put into it.”

I was hoping to use an AI story where someone made a mistake in inputs which had an almost catastrophic, or at the very minimum, a serious deviation in the expected output due to that mistake.

Either a personal story or link to one is fine.
